RESUMO

Repeat referral to adult protective services APS (recurrence) is a much-discussed topic among APS agencies as it may indicate ongoing harm, yet there is limited research examining prevalence or causes. This paper provides a foundational investigation of recurrence within a California APS county program. Drawing from thirty-three months of de-identified reports, we used logistic regression to examine the impact of intake report characteristics on repeat referral within one year after baseline case closure. One-fifth of the sample was recurrent (19.9%, n=987/4,958), with self-neglect being the most common type of report to recur (14.3%, n=307/2,141). Overall recurrence was predicted by female gender, older age, living alone, and multiple elder abuse, neglect, and exploitation (ANE) types reported at baseline, and report placed by social service provider, friends, family, landlords, and victim self-reports. Reporters personally related to the victim and social service providers are potential partners in identifying ANE, and alternate intervention approaches may be necessary.

BACKGROUND AND OBJECTIVES: The purpose of the study was to create and validate a tool that could be implemented easily to recognize the presence and assess the level of neglect in community-dwelling older adults, and to provide information about the prevalence of the phenomenon in different subgroups of older adults in Poland. RESEARCH DESIGN AND METHODS: The cross-sectional study of elder neglect and self-neglect was conducted in Lesser Poland in 2017. It included 2,443 face-to-face interviews with randomly selected community-dwelling individuals from among the general population (1,635), social service users (280), and hospital patients (528). Classical Test Theory and Item Response Theory (IRT) were used to build the scale, and its content and construct validity and reliability were assessed. RESULTS: The Self-Reported Neglect Scale (SRNS) with a 2-factor structure (basic needs and psychological needs dimensions) was created. Results of the IRT analysis showed high item discrimination (2.7-4.8 for the first factor, 0.8-3.2 for the second). The 1-year prevalence of neglect as a percentage of nonzero values of the SRNS was estimated at 11.4%. DISCUSSION AND IMPLICATIONS: The SRNS exhibited good psychometric properties. It may have promise as a tool for the assessment of neglect in epidemiological studies as well in the everyday practice of medical professionals and social workers.

RESUMO

This is a study on scientific work on the ideation, suicide attempt and self-neglect of elderly residents in LTCEs from 2002 to 2017. Documents were retrieved from the following sources: BVS/SP, SciELO, Scopus, PubMed and Web of Science, with the following descriptors: suicide attempt, suicidal ideation, self-neglect, elderly, long-term care establishment, and their correspondents in Portuguese, Spanish and French. Twenty-six papers on the subject were found. There is a consensus among the authors, whose texts are analyzed here, concerning the factors that lead the elderly to suicidal behavior: depression, illness and pain, complicated and traumatic mourning, anxiety and despair after recovery from depressive episode, poor living conditions, death of close relatives, friends, family conflicts, family history of self-inflicted events. The protection factors found are religiosity, optimistic lifestyle, satisfaction with life and investment in the autonomy and power of relationships and communication and monitored drug therapy for mental disorders such as depression.

Os fatores de proteção encontrados são religiosidade, estilo de vida otimista, satisfação com a vida e investimento na autonomia e no poder dos relacionamentos e da comunicação e tratamento farmacológico acompanhado para transtornos mentais como depressão.

AIM: The frailty index is used to measure the health status of older individuals. However, its association with self-neglect, social support and sociodemographic characteristics of older adults is underexplored. The purpose of the present study was to explore such associations among the rural older adults in Anhui Province of China, and to provide scientific evidence for policy-makers to improve the health and well-being of older adults in rural China. METHODS: A cross-sectional survey was undertaken among 3048 older adults in rural China. The frailty index was measured by an accumulation of deficits of 40 items. Ordinal logistic regression models were carried out to explore the associations of self-neglect, social support and sociodemographic characteristics with the frailty index. RESULTS: The frailty rate was 15.8% among the participants. Older adults who showed self-neglect, were female, had a lower education level, had a lower monthly income and did not drink alcohol were more prone to having frailty in the 60-76 years age group, whereas older adults who showed self-neglect, had poor social support, were female, were not smokers and did not drink alcohol were more likely to have frailty in the ≥77 years age group (P < 0.05). The interaction effect on frailty between self-neglect and social support was found in the 60-76 years age group. CONCLUSIONS: Age differences were shown in the associations between self-neglect, social support, sociodemographic characteristics and frailty index, which suggests different measures should be used accordingly for frailty prevention and management.
